It was developed at the Institute of Molecular Genetics of the Russian Academy of Sciences. It is an analogue of tuftsin, a natural peptide that regulates the immune system in the body. Selank combines this immune-modulating effect with anxiety-reducing and potentially cognition-enhancing effects [1,4].
What's interesting is how Selank works: It doesn't attach directly to receptors like classic calming medications do. Instead, it regulates gene expression in the brain - it influences which genes for messenger substances and growth factors are read. This includes BDNF (Brain-Derived Neurotrophic Factor), a kind of "fertilizer" for nerve cells that promotes their growth and networking. Selank also inhibits enzymes that break down enkephalins - these are the body's own substances that have a pain-relieving and calming effect [4,5].
How is Selank used?
The evidence-based and Russia-approved route of administration for Selank is intranasal - meaning as a nasal spray or nasal drops. Peptides would be destroyed in the gastrointestinal tract, so nasal administration bypasses the digestive tract and reaches the brain quickly. The bioavailability is 92.8%, and the effect sets in after about 5 minutes [1].
The Russian finished medication is a 0.15% solution in 3-ml vials. The approved regimen: 2-3 drops per nostril, 3 times daily, for 10-14 days. A break of 2-4 months is recommended between courses [1,2].
On the gray market (outside Russia), Selank is often sold as lyophilized powder in vials of 5 or 10 mg. Users reconstitute it themselves with BAC water and use it nasally - sometimes it is also injected subcutaneously. This practice is unproven and not validated; there are no controlled data on safety or efficacy outside the Russian approval studies. From vial to use: Mixing Selank for nasal use
If you have a Selank vial (e.g., 5 mg lyophilisate) and want to make a 0.15% solution, here's the calculation: 0.15% means 1.5 mg per ml. For 5 mg of powder, you'll need about 3.3 ml of BAC water. This gives you a solution with 1.5 mg/ml - which matches the concentration of the Russian finished medication.
A standard nasal dropper delivers about 0.05 ml per drop. At 1.5 mg/ml, one drop therefore contains about 75 mcg of Selank. How does Selank work?
Selank works through several mechanisms at once:
- BDNF modulation: It influences the Brain-Derived Neurotrophic Factor in the hippocampus and the prefrontal cortex - regions responsible for learning, memory, and emotion regulation. In animal studies, Selank protected against alcohol-related memory impairment by regulating BDNF levels [4].
- Enkephalin protection: It inhibits enzymes that break down the body's own enkephalins. These have a pain-relieving and mood-enhancing effect - by staying in the system longer, this can have an anxiety-reducing effect.
- Immune modulation: As a tuftsin analogue, Selank influences the expression of inflammation genes and cytokines under stress conditions [7,8]. This is remarkable because chronic stress weakens the immune system.
- Gene expression: It regulates genes related to GABAergic and serotonergic neurotransmission - without, however, binding directly to benzodiazepine receptors. This explains the anxiety-reducing effect without sedation.
Unlike benzodiazepines (such as diazepam or lorazepam), Selank doesn't make you sleepy, doesn't cause tolerance, and doesn't lead to withdrawal. In a Russian animal study, it even enhanced the effect of diazepam under chronic stress [5].
How effective is Selank?
The clinical evidence for Selank comes exclusively from Russia. This means: There are concrete human studies, but with a structural Russia bias and without independent top-tier Western studies.
The most important study is the Phase 3 trial by Zozulya et al. (2008), in which Selank was directly compared with the benzodiazepine medazepam in 62 patients with generalized anxiety disorder. Result: Selank was just as effective as medazepam at reducing anxiety - but without sedation, without dependence potential, and without withdrawal symptoms [1]. Further Russian studies by Medvedev et al. compared Selank with phenazepam (2014) and investigated optimizing anxiety treatment (2015) [2,3].
In animal studies, Selank also showed protective effects against alcohol-related memory impairment [4], enhanced the diazepam effect under chronic stress [5], alleviated morphine withdrawal symptoms [6], and influenced cytokine levels under social stress [8]. An fMRI study (Panikratova et al. 2020) examined functional connectivity in the brain under Selank and Semax [9].
Here's how I assess it: The Russian data are promising and plausible - especially the "anxiety-reducing without sedation" profile is attractive. But without Western replication, a residual reservation remains. Russian studies have historically sometimes had methodological weaknesses, and independent confirmation is completely lacking.

What risks and side effects are there?
In the Russian studies, Selank was well tolerated. The side effect rate was low, and compared to benzodiazepines, there was no sedation, no cognitive impairment, no tolerance development, and no withdrawal syndrome. According to the Russian prescribing information, there are no relevant drug interactions [1,2].
Still, there are important limitations: All safety data comes from Russia. There are no independent Western safety studies and no long-term data beyond 14 days of use. On the gray market outside Russia, product quality is uncontrolled - contamination, incorrect dosages, or varying purity are possible. Since September 2024, Selank has been removed from the FDA 503A list, which means US pharmacies may no longer compound it.
What is still unknown?
- No Western randomized studies exist - all human data comes from Russia.
- No independent replication of the 2008 Phase 3 results.
- No long-term safety data beyond 14 days.
- Pharmacokinetics are only rudimentarily characterized: Selank disappears from the blood within about 5 minutes, but the central effect lasts longer - exactly how that works is not fully understood.
- It's unclear whether the immune-modulating effects are relevant to the anxiety-reducing effect or are an independent additional effect.
Conclusion
Selank is one of the few peptides that exist as an approved medication somewhere in the world - namely in Russia as a nasal spray for anxiety. That sets it apart from most gray-market peptides, for which there is no approval at all. The "anxiety-reducing without sedation" profile is attractive and is supported by Russian Phase 3 data.
